PERSONALIZED SELECTION OF TERRAINKUR ROUTES BASED ON HYPERTENSIVE CRISIS PREDICTION IN PATIENTS WITH OVERWEIGHT/OBESITY AND HYPERTENSION
Abstract
Aim. This study aims to evaluate the effectiveness and safety of a prognostic model for meteoropathic reactions (hypertensive crisis, HC) for selecting optimal therapeutic walking routes (terrainkur) in patients with overweight (obesity) and hypertension. Materials and methods. This open, prospective, controlled clinical study was conducted at the Vulan health resort from April 1 to October 31, 2023, and involved 95 patients with overweight (obesity) and hypertension. In addition to standard therapy and a basic health resort treatment program, patients received 10 therapeutic walking (terrainkur) sessions. Group 1 (G1) walked in a mild microclimate, Group 2 (G2) – in a training microclimate, and Group 3 (G3) followed
a route selected by a computer-based predictive model for hypertensive crisis. Pre- and post-intervention assessments included heart rate variability, hemodynamics (Zdorovye-Express, Russia), body composition (Medass, Russia), the Luscher color test, the Well-Being-Activity- Mood (WAM) questionnaire, and the six-minute walk test. Results. G3 demonstrated the most pronounced reduction in fat mass (р > 0.05, Wilcoxon test). Moreover, this group showed positive changes in HRV parameters, with decreases of –3.9% in the functional changes index (р > 0.05, Wilcoxon test) and –28.6% in the regulatory activity index (р > 0.05, Wilcoxon test). These changes were statistically significant compared to G1 and G2. Conclusion. The safest and most effective terrainkur is one that alternates microclimate regimes based on the computer program’s recommendations.
